To establish a clinical MOU with a member hospital of the Georgia Hospital Association, the Office of Legal Affairs will need these three forms filled out:

Authorization for Release of Records and Information

Student Applied Learning Experience Agreement

Faculty Supervision Applied Learning Experience Agreement

Please provide these forms at the time you submit your request to establish a clinical MOU.
