Dear Eagle Incentive Program (EIP) Students and Parents :

On behalf of the Staff of the Georgia Southern University Health Services, congratulations on your acceptance into the Eagle Incentive Program and welcome to the University. We look forward to serving you this summer and thereafter once you successfully complete the EIP.

Location & Hours of Operation : The Health Center facility is located on Forest Drive across from the Watson Hall residence complex and near the Lakeside Café. Operating hours are 8:00 a.m. to 5:00 p.m., Monday through Friday during summer sessions, and the fall & spring semesters; and 10 a.m. to 4:30 p.m. during break periods.

Staffing & Services : We provide a full range of primary care services staffed by experienced, fully licensed physicians, nurse practitioners, nurses, and clinical and administrative support staff. In addition to regular office visits, services include women's health, immunization/allergy injection, radiology, clinical laboratory, pharmacy, respiratory therapy, and health education & wellness promotion. The majority of the services are supported through a summer session health fee and a semester fee for the fall and spring terms. Certain specialty care services, vaccinations, medical equipment, and supplies require discounted charges.

Web-based Services : In addition to the quality medical services mentioned above, Health Services is pleased to offer MedBuddy-U , a secure, private internet communication tool for students that will make access to healthcare at the Health Center more convenient and accessible, including appointment requests, prescription refills, and secure messaging to/from medical provider(s). EIP students should enroll in and begin using MedBuddy-U to communicate with Health Services staff. My Student Health Zone is a web-based health and wellness information service designed exclusively for university students. Both of these services are available by going to the http://services.georgiasouthern.edu/health Web page and clicking on the MedBuddy-U and My Student Health Zone icons.

Immunization Compliance : The Board of Regents of the University System of Georgia requires all new students, including EIP students, to provide proof of immunization against Measles, Mumps, and Rubella (MMR); Tetanus-Diphtheria; Varicella (chickenpox); Hepatitis B (three doses of vaccine or positive Hepatitis surface antibody), required of all students who are 18 years of age or younger; and for residents of University Housing, acknowledgement of vaccination against Meningococcal disease, or acknowledgement of information received about Meningococcal disease and declined offer to be vaccinated before they are allowed to register for or attend classes at Georgia Southern University. Health Services administers this policy and will gladly help students with appropriate forms and information.

EIP students are urged to acquire and document these immunizations before arriving at Georgia Southern. Forms and additional information specific to your immunization compliance requirements is available through the following Immunization and Medical History forms. Additional information on the EIP can be found at the following Admissions Office link: http://admissions.georgiasouthern.edu/EIP.htm

Respectfully Yours,
