
Supervisor Checklist for New EmployeesUse this checklist to request computer access for new employees. If all activities are completed by the supervisor and the new employee in a timely fashion, it’s possible to have access already established when the employee arrives on campus. With good response from the employee in turning around signed forms, this process can typically be completed within 5 business days. This process works like this: You must access the link below and fill out the online form for your
new employee. Basic information for the employee shown at the top of the form must be
provided so that we can complete setup for the employee. Select those items your new employee will need upon arrival to campus
on the bottom half of the form. Submit the form by clicking ‘Send this info’ at the bottom. If you choose certain items on the form, the new employee will have to sign specific documents acknowledging his/her understanding of University policies. If you wish to expedite this process and have the accounts ready upon the new employee’s arrival, you may send the employee these links and request that he/she follow instructions on the form(s) to complete the process.
