- GRADUATE ASSISTANTS, INTERNATIONAL & NURSING STUDENTS AND THE MANDATORY PLAN -

Each semester period (fall and spring/summer) you will need to either enroll in, or waive out of, the mandatory plan through the Pearce & Pearce Georgia Southern online program.

The information you provide, particularly the e-mail address you use most often, will be used to create your own login page which you can go back into to print our a temporary ID card and do network provider searches. Information from the Insurance Program Administrator, Pearce & Pearce, will also be sent to you via this secure Website.

- 2008-2009 ACADEMIC YEAR: PLAN ENROLLMENT INFO & PREMIUM RATES -

Open Enrollment using the Pearce & Pearce on-line enrollment process for the Fall semester begins 8/1/2008 and will end on 9/1/2008. The Mandatory Plan(s) on-line waiver process will end on 9/1/2008.

The Board of Regents of the University System of Georgia has renewed its contract with Pearce & Pearce, Inc. to provide student health insurance for the 2008-2009 academic year (August 1, 2008 – July 31, 2009). All 35 Institutions of the University System of Georgia (USG) are participating in the Student Health Insurance Plan (SHIP) administered by Pearce & Pearce, Inc. For the 08-09 school year, there will be two different Mandatory Plans for students required to have insurance. Those two plans will be Graduate Mandatory and Undergraduate Mandatory. If you are part of these Mandatory Groups you will have the insurance premium charged to your tuition bill. Students in these mandatory groups will have the option to waive the student insurance if they have a pre-approved insurance plan. The Voluntary Plan will remain an option for students who are not required to have insurance.

Mandatory Groups

1. Undergraduate and Graduate International Students holding “F” or “J” visa status.
2. Undergraduate and Graduate Students enrolled in academic programs that require proof of health insurance.
3. Graduate Students receiving Qualified Graduate Assistantships.
4. Graduate Students receiving Qualified Fellowships.
5. Graduate Students receiving Qualified Training Grants
Graduate Assistants: Those students who hold any type of graduate assistantship of at least 1/3 time. This includes general graduate assistantships, teaching assistantships, laboratory assistantships, and research assistantships, regardless of the source of funds; as well as Graduate School Assistantships and Graduate Recruitment Opportunity (GRO) assistantships.
Fellowship Recipients: Graduate students supported by Fellowships administered by the Institution that provide funding for the student in an amount equal to or greater than the prevailing in-state tuition rate for graduate students. Such Fellowships include, but are not limited to, Institution Presidential Graduate Fellowships, National Research Service Awards, National Science Foundation Fellowships, and other state and federally funded Fellowships.
Departmental Training Grants: Graduate students supported on state or federally funded training grants administered by the Institution that provide funding for the student in an amount equal to or greater than the prevailing in-state tuition rate for graduate students.

Undergraduate Mandatory Student Plan

Rates for Fall Semester: $358.00 Rates for Spring/Summer Semester: $472.00

Voluntary Student Plan (all students who are not part of the mandatory groups)

Rates for Fall Semester: $581.00 Rates for Spring/Summer Semester: $770.00

REFUNDS ARE NOT ALLOWED unless the Covered Student enters full time active duty in any Armed Forces*.
Insurance Premium will appear on the Covered Student’s tuition bill (for mandatory undergraduates and mandatory graduates) unless he or she shows proof of other insurance and waives coverage under this plan.
(*Excludes Reserve or National Guard duty for training unless it exceeds 31 days. Submit proof of service to receive a pro-rata refund of premium for this period.)

- EXPRESS SCRIPTS PHARMACY -

An additional service offering provided by Pearce & Pearce in this years plan is the Express Scripts Pharmacy Program.

The Express Scripts System will be used in 2007-2008 to allow the Health Service University Pharmacy and its off-campus parent pharmacy – Forest Heights Pharmacy – to file USG SHIP prescription claims electronically. This will allow USG SHIP covered Georgia Southern students to fill their prescriptions at these two pharmacy locations without having to pay and submit claims for reimbursement
